IconicCare is hiring a full-time physician extender (NP/PA) to oversee clinical care and facility relationships across CA facilities. You will provide 2-3 days of rounds at your designated facility and coordinate with primary care teams to optimize therapy participation and functional recovery.

We are seeking a passionate, motivated provider to join us full-time and independently offer care and consultations at our contracted facilities.
Must Live in: San Francisco, CA; Antioch, CA; Tracy, CA; Oakland, CA or Sacramento, CA
Sacramento, Brentwood, San Leandro or Hayward areas preferred
This role would see either see facilities from San Jose to Oakland weekly, and facilities in Sacramento monthly or vice versa, depending on candidate home location, with compensated mileage.

Primarily responsible for regional oversight of clinical care and facility relationships, you will also round 2-3 days a week at your own designated facility to care for patients who are experiencing difficulty participating in therapy due to pain or functional limitations.
This role is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.
We’re looking to make hiring decisions soon, with a start date in 4-6 weeks following insurance and Medicare credentialing.
