Job summary

Chestnut Health Company in Indiana is seeking a Nurse Practitioner to join our physiatry team. This role focuses on rehabilitation and improving functional outcomes through collaborative care with therapists and nurses. The ideal candidate is licensed in the state, holds a NP certification (ANCC or AANP), has DEA and CS licenses, and is eager to work in Physiatry.

Qualifications

Actively licensed to practice in the state of employed
Current DEA License (required upon start date)
ANCC or AANP certification required
Desire to work in Physiatry/PM&R
Orthopedic or Skilled Nursing experience is a plus
Physiatry experience is a plus

Responsibilities

Collaborate with therapists, nurses, and other healthcare professionals to develop and implement comprehensive treatment plans for patients in rehabilitation
Conduct thorough patient assessments, including medical histories, physical examinations, and diagnostic evaluations
Monitor patient progress and adjust treatment plans to optimize functional outcomes and independence
Participate in multidisciplinary team meetings, case conferences, and educational activities
Maintain accurate documentation of evaluations, plans, progress notes, and discharge summaries in EMR
Educate patients and families regarding rehabilitation process, expectations, and post-discharge care
