Job summary

Talented-Medical-Solutions in Alameda, California is seeking a Travel Physical Therapist (PT) for a 15-week contract. This position requires an active California PT license, a recent clinical PT background, and strong assessment and documentation skills to support patient-centered rehabilitation.

You will implement individualized treatment plans, provide therapeutic exercises and manual therapy, educate patients and caregivers, and collaborate with a multidisciplinary team to promote mobility

Qualifications

• Active Physical Therapist license eligible to practice in California.
• Graduate of an accredited Physical Therapy program.
• Current BLS certification required by facility.
• Recent clinical Physical Therapy experience.

Responsibilities

• Evaluate patients' physical abilities, functional limitations, and rehabilitation needs.
• Develop individualized treatment plans based on patient goals and findings.
• Provide therapeutic exercises, manual therapy, mobility training, and related interventions.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers on exercises and injury prevention.
• Document evaluations, treatment sessions, progress and discharge plans accurately.
• Collaborate with physicians, nurses, and other professionals as part of the care team.
• Promote patient safety and ensure care aligns with standards and policies.

Position Overview

We are seeking a skilled and compassionate Travel Physical Therapist (PT) to join a healthcare team in Alameda, California.

This is an exciting 15-week contract opportunity for a motivated Physical Therapist who is passionate about helping patients restore mobility, improve functional independence, and achieve meaningful rehabilitation goals.

The ideal candidate will bring strong clinical judgment, excellent communication skills, and a patient-centered approach to every treatment session.

Evaluate patients' physical abilities, functional limitations, and rehabilitation needs.

Develop individualized treatment plans based on patient goals and clinical findings.

Provide therapeutic exercises, manual therapy, mobility training, and other appropriate interventions.

Monitor patient progress and modify treatment plans as needed.

Educate patients and caregivers on exercises, mobility techniques, and injury prevention.

Document evaluations, treatment sessions, progress, and discharge plans accurately.

Collaborate with physicians, nurses, occupational therapists, and other members of the care team.

Promote patient safety and functional independence throughout the rehabilitation process.

Ensure all care is delivered in accordance with professional standards and facility policies.
