As a PHSO Resource Coordinator, you will play a pivotal role in coordinating care for our primary care population. Your responsibilities will include:
Utilizing data and reporting to identify patient care needs.
Proactively reaching out to patients to engage in primary care services including appropriate visits, labs or screenings.
Collaborating with PHSO nurses and primary care teams to ensure patient needs are being met.
Acting as a central point of contact with external parties including payer partners and community providers when applicable.
Maintaining accurate and up to date electronic health records including documentation of key services and screenings.

QUALIFICATIONS
Education: Associates Degree in Human Services or 2-3 years of experience in a healthcare setting (or equivalent). Bachelor’s level degree in Human Services preferred.
Experience: Care coordination or other transferable skills preferred. Customer service preferred.
Skills: We are looking for a proactive and reliable candidate who requires minimal oversight. Strong communication skills are essential, and applicant must be comfortable making and receiving phone calls. It is important for the applicant to be collaborative in nature and excited to become a part of a cross-functional team.
Location: Onsite presence required at the IDX Building in South Burlington, VT with flexibility to work remotely at times.
