Job summary

Community First Health Plans is seeking a Manager, Population Health Management RN to lead and supervise PHM initiatives and oversight of short-term case management therapy services for CFHP members.

The role requires collaboration with staff at all levels of the health plan to improve member satisfaction and operational efficiency. A Master’s Degree in Speech Language Pathology and management experience are required, with HMO experience preferred.

Qualifications

• Master’s Degree in Speech Language Pathology.
• Minimum of three years of clinical experience with two years of management experience is required.
• Referral management experience is required.
• Working knowledge of UM criteria ( and health care coding ICD-9, CPT, HCPCS, DRG) is required.
• HMO experience is preferred.

Responsibilities

• Manages, supervises and assists the Director, Population Health Management with oversight of short-term case management therapy services to facilitate high quality, cost effective care for CFHP members.
• Works with staff at all levels of the health plan to facilitate member satisfaction and efficiency.
