Job summary

Community First Health Plans is seeking a Manager, Population Health Management RN to lead and supervise PHM initiatives and oversight of short-term case management therapy services for CFHP members.

The role requires collaboration with staff at all levels of the health plan to improve member satisfaction and operational efficiency. A Master’s Degree in Speech Language Pathology and management experience are required, with HMO experience preferred.

Qualifications

• Master’s Degree in Speech Language Pathology.
• Minimum of three years of clinical experience with two years of management experience is required.
• Referral management experience is required.
• Working knowledge of UM criteria ( and health care coding ICD-9, CPT, HCPCS, DRG) is required.
• HMO experience is preferred.

(Community First) membership. Works with staff at all levels of the health plan to facilitate member satisfaction and efficiency.

EDUCATION/EXPERIENCE
Master’s Degree in Speech Language Pathology. Minimum of three years of clinical experience with two years of management experience is required. Referral management experience is required. Working knowledge of UM criteria ( and health care coding (ICD-9, CPT, HCPCS, DRG) is required. HMO experience is preferred.

LICENSURE/CERTIFICATION
A current license to practice professionally as a Speech Language Pathologist (SLP) issued by the Texas Department of Licensing and Regulation for Speech-Language Pathology and Audiology is required.
