Qualifications

• Minimum 1 year of experience supervising PHC/PAS or related home health care.
• Strong communication, organization, and leadership capabilities.
• Must have a valid driver’s license, reliable transportation, and proof of insurance.
• Proficiency with Microsoft Office, EVV systems, and EMR documentation preferred.

• Monitor attendant performance and documentation.
• Provide training, coaching, and performance feedback to attendants to promote best practices in client care.

• Assist with incident reporting, plan of care updates, and annual reassessments.
• Ensure all field activities meet HHSC and agency compliance standards.
• Maintain accurate documentation and submit reports promptly.
• Support recruitment and retention efforts for qualified attendants.
• Serve as a liaison between the office and field staff, promoting communication and teamwork.

Position Summary

The PHC Field Supervisor oversees and coordinates the delivery of Personal Assistance Services (PAS) to clients in accordance with state regulations, agency policies, and care plans. This role involves conducting supervisory visits, training attendants, maintaining communication with clients and caregivers, and ensuring compliance with HHSC and agency standards.

Key Responsibilities

• Conduct initial, supervisory, and follow-up home visits with PHC clients to ensure quality and satisfaction of care.
• Monitor attendant performance, & documentation
• Provide training, coaching, and performance feedback to attendants to promote best practices in client care.
• Coordinate withoffice staff to ensure accurate service delivery and timely reporting.
