Description:
Supports the Trinity Health Alliance of Michigan pharmacy team to enhance workflow for clinical pharmacy services throughout the network, including medication therapy management (i.e. medication reviews and chronic disease management) and medication adherence. Provides patient identification, outreach and scheduling support for Pharmacists completing clinical services within the network and providing telephonic patient care (i.e. pre-visit med rec, adherence monitoring) and supporting data/metric tracking, trending and reporting.
The Population Health Pharmacy technician supports the Trinity Health Alliance of Michigan pharmacy team to enhance workflow for clinical pharmacy services throughout the network.
Primary actives include outreach and scheduling for comprehensive medication management services with network pharmacists & outreaching to patients to educate and support them with medication adherence.
This is a remote position, but the ideal candidate will be located in Michigan.
Education
Required: High School Diploma/GED/Equivalent
Preferred: Post-secondary education (vocational, technical, diploma program, bachelors, etc.)
Credentials/Licensure
Required: Michigan Board of Pharmacy Technician registration/license; Certified Pharmacy Technician
(CPhT) with PTCB.
Preferred: Graduate of formal pharmacy technician training program; BLS certification.
Related Experience
Required: Three years of pharmacy technician practice
