Job summary

Qualifications

• Education: High school diploma or equivalent.
• Experience: 2+ years Pharmacy Technician experience; 1 year med history interviews may substitute.
• Certification: AR Pharmacy Technician license and CPhT or must obtain within 6 months.
• Other: Knowledge of outpatient doses; strong interpersonal and written communication; operate pharmacy equipment.

Responsibilities

• Perform medication history assessments for new admissions.
• Provide complete information for each med (drug, dose, route, indication, last dose).
• Verify patient allergy information and document accurately.
• Coordinate medication history with the nursing team.
• Document history and allergies in the EHR timely.
• Precept technicians, students, pharmacists, and nurses on home med interviews and documentation.
• Function as a licensed pharmacy technician.
