Job Overview

As a Pharmacy Technician Medication History Specialist, you will perform medication histories for new hospital admissions. Your responsibilities include collecting accurate medication information, verifying patient allergies, and updating records under supervision of a clinical pharmacist. Your schedule may include weekends, evenings, holidays, and on-call hours.

Key Responsibilities
  1. Perform medication histories through patient and family interviews, and contact external healthcare providers as needed.
  2. Update medication, allergy, and healthcare provider information in electronic health records accurately and promptly.
  3. Document interventions and communicate unresolved issues with pharmacy specialists.
  4. Assist with medication reconciliation and patient education preparations.
  5. Participate in staff meetings and adhere to scheduling requirements.
Qualifications
  • High School diploma or GED
  • At least 2 years of retail pharmacy experience
  • Preferred inpatient pharmacy experience
  • Current pharmacy technician registration or intern license
  • Certification as a Pharmacy Technician or within 90 days of hire
