Job Summary and Qualifications

As a Pharmacy Technician II is responsible for accurately performing all advanced and basic pharmacy technician functions in a timely manner. A core focus of this role involves spearheading inpatient medication reconciliation to ensure patient safety upon admission. In addition, this position is responsible for performing duties associated with Automated Dispensing Cabinets (ADCs) and other specialized duties as assigned by the pharmacy management team. In this role:

Inpatient Medication Reconciliation (Primary Responsibility)
  • Patient Interviews: Conduct direct, professional patient interviews on inpatient units to obtain comprehensive and accurate home medication histories.
  • Clinical Verification: Verify medication names, dosages, frequencies, formulations, and the precise time of the last taken dose.
  • Discrepancy Resolution: Investigate gaps in medication histories by actively contacting community pharmacies, primary care providers, or reviewing external prescription databases.
  • EHR Documentation: Accurately input and update the validated home medication list within the Electronic Health Record (EHR) to provide attending physicians with reliable data for safe admission prescribing.
Core Pharmacy Operations
  • Medication Preparation: Accurately package and prepare medications for dispensing while maintaining strict medication storage and security standards.
  • ADC Management: Perform daily Automated Dispensing Cabinet routines, including restocking, auditing, and maintenance as assigned.
  • Hospital Support: Safely transport medications and supplies to various areas of the hospital, interacting collaboratively with hospital personnel both in person and by phone.
  • Cost Management: Help manage drug supplies in a cost-effective manner and proactively identify opportunities to minimize medication waste.
  • Leadership: Mentor new technicians and assist the Pharmacy Director with special departmental projects as needed.
What qualifications you will need:
  • (CPhT) Certified Pharmacy Technician
  • (PharmTech) Pharmacy Technician License
  • One (1) year experience required (Experience with patient interviewing or medication history taking is highly preferred)
