Job summary
Legacy Health is seeking a Pharmacy Technician to join our Pharmacy Services team, working under a pharmacist to support patients across care transitions with compassion and accuracy. You will collect the best possible medication history, aid in discharge medication provisioning, and collaborate with pharmacists, nurses, physicians, and other care teammates to ensure safe, effective medication use.
Requires a high school diploma or equivalent; pharmacy technician program is preferred; minimum 2
Qualifications
- Must have high school diploma or equivalent; pharmacy tech program preferred.
- Minimum 2 years as a pharmacy technician; retail or inpatient hospital experience preferred.
- Basic familiarity with computers and navigating electronic systems.
Responsibilities
- Work under pharmacist supervision to support patients through care phases.
- Collect best possible medication history for accurate reconciliation.
- Facilitate discharge medications to ensure access post-hospitalization.
- Collaborate with pharmacists, nurses, physicians, and care team for safe med use.
- Support efforts to reduce medication-related readmissions and improve continuity of care.
