UnityPoint Health is seeking a Pharmacy Support Assistant to obtain medication histories at hospital admission, interviewing patients or caregivers to document current medications and to verify information with pharmacies and providers. You will support pharmacists with timely communication and accurate data collection in a fast-paced hospital setting.

Qualifications

• High school diploma or equivalent is required.
• Previous Pharmacy or healthcare experience is preferred.
• Register as a support person with Board of Pharmacy within 30 days of hire.

Responsibilities

• Obtain accurate and up-to-date medication histories upon hospital admission.
• Interview patients and/or caregivers to gather detailed information about current medications.
• Contact retail pharmacies, outpatient providers, and other institutions to verify and clarify medication information.

The Pharmacy Support Assistant provides essential administrative and operational support to ensure the smooth and efficient functioning of the pharmacy. This role plays a critical role in ensuring safe and effective patient care by obtaining accurate and up-to-date medication histories upon hospital admission. This position is responsible for interviewing patients and/or caregivers to gather detailed information regarding current medications, including prescription drugs, over-the-counter products, and supplements. In addition, the Pharmacy Support Assistant will contact retail pharmacies, outpatient providers, and other healthcare institutions as needed to verify and clarify medication information. The information collected will be documented and communicated to the pharmacist for review, supporting providers in making informed clinical decisions.

Strong communication skills, attention to detail, and the ability to interact professionally with patients and healthcare partners are essential for success in this role.
