Job summary

MUSC Health Partners (MHP) seeks an ambulatory pharmacist embedded in a primary care clinic to collaborate with physicians, APPs, nursing staff, and other healthcare professionals. The role focuses on improving patient outcomes by managing medication therapy, providing patient education, and addressing preventive care needs.

Responsibilities include completing annual wellness visits, closing care gaps, and supporting a team-oriented ambulatory care environment within a university medical system.

Qualifications

• Doctor of Pharmacy or a Bachelor's degree in pharmacy.
• Completion of a pharmacy residency preferred.
• Licensed as a Registered Pharmacist by the South Carolina Board of Pharmacy.
• Current American Heart Association (AHA) Basic Life Support (BLS) certification or American Red Cross BLS for Healthcare Providers certification is required

Responsibilities

• Manage medication therapy as part of ambulatory pharmacy services.
• Provide patient education to optimize therapy and outcomes.
• Complete annual wellness visits and close care gaps.

Additional Job Description

Minimum Training and Education: Doctor of Pharmacy or a Bachelor's degree in pharmacy. Completion of a pharmacy residency preferred.
