As a member of the ambulatory pharmacy services team, you will be embedded in a primary care clinic working alongside physicians, advanced practice providers, nursing staff, and other healthcare professionals. You will play an important role in improving patient outcomes by managing medication therapy, providing patient education, completing annual wellness visits, closing care gaps, and other related responsibilities.
