Adventist Health Clear Lake is seeking a Pharmacist for a 13-week travel assignment in Clearlake, CA.

The position features a Day Shift 4x10s schedule from 6:30 am to 5:30 pm, with 40 hours per week, and involves inpatient and ambulatory care settings.

Must be a graduate of an accredited college of pharmacy with CA Pharmacist License; hospital pharmacy experience and residency preferred. Union representation applies.

Responsibilities

• Performs and is responsible for the day-to-day professional activities of the Pharmacy Department including compounding, admixing sterile products, drug consultations, dispensing, providing drug information, and monitoring patient drug therapy.
• Interacts with physicians and nurses regarding orders and questions.
• Maintains computerized patient records and monitors drug therapy for safety and efficacy.
• Maintains daily records of controlled substances.
• Reviews inventory and ensures outdated drugs are removed.
• Supervises support personnel in IV admixtures and dispensing.
• Provides clinical drug information to meet the needs of physicians and nursing staff.
• Supports inpatient and ambulatory areas including ambulatory clinics and operating rooms.

Description:

• Performs and is responsible for the day-to-day professional activities of the Pharmacy Department including duties involving compounding, admixing sterile products, drug consultations, dispensing, providing drug information, and monitoring patient drug therapy.
• Job is represented by a union.
• Conducts and accurately documents in the medical record drug protocols as ordered by physicians, including but not limited to TPN, Pharmacokinetic, and Heparin.
• Contacts and intervenes with physicians and/or nurses regarding questions, errors, or irregularities on orders.
• Maintains and meets expectations on time for all competencies, licenses, certifications, and education requirements as outlined by local administration and regulatory agencies.
• Maintains computerized patient record system to monitor medication therapy for safety and efficacy, including but not limited to medications, medication errors, and nursing unit inspections.
• Maintains daily records of controlled drugs as required by law.
• Monitors and intervenes on all drug regimens as indicated by the quality assurance plan, including but not limited to drug-drug interactions, drug allergies, toxic/therapeutic drug levels, and appropriateness indications.
• Provides clinical drug information to meet the needs of the physicians and nursing staff through direct contact and/or committee participation.
• Reviews inventory levels to ensure that all outdated drugs or drugs that have lost their potency are removed.
• Supervises support personnel in routine performance of their duties, including but not limited to IV admixtures, medication dispensation, and medication compounding.
• Willingly performs other duties and innovations as assigned.
• This department controls, mixes, compounds, and distributes drugs, and provides pharmaceutical consultation in support of inpatients, as well as to ambulatory areas including ambulatory/outpatient clinics, infusion therapy, infusion oncology, ambulatory dialysis, ambulatory surgery, and operating rooms.
• Prescription-based retail entities are excluded.
