Job summary

Adventist Health is seeking a Pharmacist (AHCL) to manage daily hospital pharmacy operations, including compounding, dispensing, drug information, and therapy monitoring. This role requires California licensure, hospital pharmacy experience, and the ability to interface professionally with staff and patients.

The Pharmacist will oversee drug protocols, maintain records, and ensure compliance with Joint Commission and CMS standards, while supervising support staff and promoting medication safety.

Qualifications

  • Hospital pharmacy practice experience preferred.
  • Residency training preferred.
  • Current California Pharmacist License required or eligible.
  • Interacts with hospital staff and the public with professionalism and discretion.

Responsibilities

  • Conduct and document pharmacotherapy interventions and drug protocols as ordered by physicians.
  • Maintain timely competency, licenses, and education requirements per AH, TJC, CMS, and regulators.
  • Maintain computerized patient records and monitor medication therapy for safety and efficacy.
  • Maintain daily records of controlled substances.
  • Monitor for drug interactions, allergies, and therapeutic appropriateness.
  • Provide clinical drug information to physicians and nursing staff.
  • Review inventory to remove outdated or degraded medications.
  • Supervise support personnel in IV admixtures, dispensing, and compounding.

Education

Graduate of an accredited college of pharmacy
Residency training preferred
Current California Pharmacist License

  • EDUCATION AND EXPERIENCE: Experience in hospital pharmacy practice preferred
  • EDUCATION AND EXPERIENCE: Graduate of an accredited college of pharmacy

  • LICENSES OR CERTIFICATIONS: Current California Pharmacist License

  • PHYSICAL REQUIREMENTS (a): This job requires frequent bending, squatting, kneeling, climbing, reaching above shoulders, sitting, walking inclines and declines, standing, talking, hearing, and performing repetitive hand motions. Vision requirements include close vision and the ability to adjust focus. The employee will occasionally lift boxes and/or supplies weighing up to 25 pounds. The employee must demonstrate lifting a 25-pound box from the floor.
  • POTENTIAL EXPOSURE TO BLOOD & BODY FLUIDS: Category 3 for potential exposure to blood/body fluids. (Does not require the performance of procedures or other tasks in the work routine that involve exposure to blood, body fluids or tissues, and the assisting in cases of emergency medical care or first aid is not a requirement of employment. Tasks that involve handling of implements or utensils, use of the public or shared bathroom facilities, telephones, and personal contacts such as handshaking, are all considered Category 3 tasks.)
Job Roles: Pharmacist (AHCL)
  • Conducts and accurately documents in the medical record drug protocols as ordered by physicians including, but not limited to, TPN, Pharmacokinetic, and Heparin. Contacts and intervenes with physicians and/or nurses regarding questions, errors, or irregularities on orders.
  • Maintains and meets expectations on time for all competencies, license, certifications and education requirements as outlined by local administration, Adventist Health (AH), The Joint Commission (TJC), Centers for Medicare and Medicaid Services (CMS), and all other regulatory agencies.
  • Maintains computerized patient record system to monitor medication therapy for safety and efficacy, including but not limited to, medications, med errors, and nursing unit inspections.
  • Maintains daily records of controlled drugs as required by law.
  • Monitors and intervenes on all drug regimens as indicated by quality assurance plan, including, but not limited to, drug-drug interactions, drug allergies, toxic/therapeutic drug levels, and appropriateness indications.
  • Provides clinical drug information to meet the needs of the physicians and nursing staff through direct contact and/or committee participation.
  • Reviews inventory levels to ensure that all outdated drugs, or drugs that have lost their potency, are removed.
  • Supervises support personnel in routine performance of their duties, including but not limited to, IV admixtures, medication dispensation and medication compounding. Willingly performs other duties and innovations as assigned.
