Job summary

Consonus Pharmacy Services, LLC is seeking an Admissions Specialist I to ensure patient data in our electronic health records is accurate and up to date. You will enter and verify new patient information, navigate ADT admissions, and support transitions within a long-term care environment.

The role requires strong data-entry skills, proficiency with MS Office, excellent communication, and the ability to multi-task in a fast-paced setting, with rotating weekends included.

Responsibilities

• Evaluate level of care and load short stay dates when applicable.

Consonus Pharmacy is a closed‑door long‑term care pharmacy specializing in the needs of nursing facilities, assisted living residents, and their caregivers. We are currently hiring an Admissions Specialist to join our team!

Overview

This role will ensure our data in our electronic health records on patient information is accurate and up to date. The ideal candidate is a person who thrives on ensuring accuracy and has proficient data entry skills.

As the Admissions Specialist your responsibilities will include, but are not limited to:

• Complete all manual admissions
• Navigate multiple electronic health records – ADT integrated admissions
• Enter and verify all new patient information
• Update patient changes i.e. rooms, beds, status as needed
• Evaluate level of care and load short stay dates when applicable
• Basic general insurance review (E1D, E1 Commercial and Medicaid)
• Customer Agreement processing
