Job summary

Overview

Pharmacist – Transitions of Care

The Transitions of Care Pharmacist provides comprehensive medication management to support safe and effective transitions across the continuum of care. This role performs medication reconciliation for patients upon admission and discharge, identifies and resolves medication-related problems, and collaborates with physicians, nurses, and other members of the healthcare team to optimize medication therapy.

Key Responsibilities

• Perform comprehensive medication reconciliation for patients at admission and discharge.
• Identify medication discrepancies, drug-related problems, and opportunities to optimize therapy.
• Develop and communicate medication recommendations to providers and care teams.
• Provide patient and caregiver education to promote medication understanding, adherence, and safe use after discharge.
• Support discharge medication planning, including medication access, affordability, and coordination with outpatient pharmacies when appropriate.
• Facilitate safe transitions between inpatient, outpatient, and post-acute care settings.
• Document clinical interventions and medication reconciliation activities in the electronic medical record.
• Participate in initiatives designed to reduce medication-related adverse events and hospital readmissions.
• Collaborate with interdisciplinary teams to improve the quality, safety, and efficiency of the medication-use process.

Qualifications

• Minimum Education and Experience Requirements:
  • A. Education: Graduation from an accredited school or college of pharmacy.
  • B. Experience and Training: 1. Previous experience in contemporary hospital pharmacy practice is highly desirable. 2. Strong communication skills. 3. Ability to positively interact with professional and support personnel in the health science center. 4. Pharm.D. degree and/or completion of ASHP residency is highly preferred.
• Motor Vehicle Operator Designation: Employees in this position: Will not operate vehicles for an assigned business purpose NOTE: A frequent driver is defined as one who uses his/her personal or Shands automobile a) at least once daily, b) at least five individual trips per week or c) drives, on average, over 150 miles per week in the performance of his/her job.
• Licensure/Certification/Registration: Registered pharmacist in the state of Florida or eligible for same.
• Additional Requirement: As a condition of employment within the Pharmacy Services Department of Shands Hospital all employees must be accessible by telephone. This requires each employee to provide a telephone number to the department Staff Assistant, in order for the department to reach them in the event of hospital need and/or emergency.
