Candidate must have a State license and EPIC experience, with strong skills in medication reconciliation, discharge counseling, and antimicrobial stewardship to support nationwide patient care.

Qualifications

• Must have State License to Apply.
• EPIC experience required.
• Experience with medication reconciliation for discharge medication lists.
• Communication skills for gathering prior-to-admission home medication lists from patients, families, or pharmacies.
• Communication skills for discharge counseling with patients and families.
• Clinical knowledge of antimicrobial stewardship.
• Experience interpreting culture results for recommendations.
• Knowledge of UTI, pneumonia, and SSTI for recommendations.
• Experience with pharmacokinetic dosing management and monitoring.
• Experience with anticoagulation dosing management and monitoring.
• Schedule: 5x8.

Responsibilities

• Provide clinical expertise to ensure the safe and thoughtful distribution of medications.
• Gather and review prior-to-admission home medication lists from patients, families, or pharmacies.
• Conduct discharge counseling with patients and families.
• Apply antimicrobial stewardship recommendations.
• Interpret culture results to guide therapy.
• Oversee pharmacokinetic dosing management and monitoring.
• Oversee anticoagulation dosing management and monitoring.
