Job summary

CommUnityCare is seeking a Pharmacist to join a community/retail pharmacy team in Texas. You will ensure safe dispensing, review patient regimens, provide counseling, and maintain required records while collaborating with providers and staff to support patient-centered care.

The role requires a PharmD or Bachelors in Pharmacy, 1-5 years of relevant experience, Texas license, and CPR certification. A strong attention to detail and communication skills are essential for success.

Qualifications

• Bachelor’s degree in Pharmacy or PharmD required.
• 1-5 years of experience in retail or mail order pharmacy.
• Current Texas State Board of Pharmacy license.
• CPR certification (American Heart Association or Red Cross) required and maintained.

Overview

In collaboration with patients, families (as defined by the patient) and staff across all disciplines and departments is responsible for providing pharmaceutical care services.

Responsibilities

PRIMARY TASKS & DUTIES

• Ensure that prescriptions are dispensed safely and accurately as prescribed.
• Conduct patient drug regimen review.
• Provide patient prescription counseling services as required.
• Prepare and maintain required records as per requirements.
• Communicate with patients, providers and other clinical staff as needed.
• Perform other duties as assigned.

• Ensure all tasks provided and associated with patient care, patient administrative processes and related duties comply with all regulatory and accreditation standards including The Joint Commission and CommUnityCare Standard Operating Procedures and CommUnityCare Policies and Procedures.

• Complete all mandatory training and maintain appropriate credentials/licensure.

ESSENTIAL FUNCTIONS/ KEY COMPETENCIES

• High level of skill at building relationships and providing excellent customer service.
• High degree of knowledge of drug therapy and dispensing medications.
• Basic computer systems knowledge, including but not limited to Microsoft Office products.
• Strong attention to detail and accuracy.
• Excellent verbal and written communication skills.

Qualifications

EDUCATION:

• Bachelor’s degree in Pharmacy or Doctor of Pharmacy (PharmD) required.

EXPERIENCE:

• 1-5 years of experience in retail (community) or mail order pharmacy required.

REQUIRED CERTIFICATIONS/LICENSURE:

• Current license by Texas State Board of Pharmacy required.
• Current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red Cross required. (Will be required to maintain a current CPR certification during employment)
