Job summary

Kindred Hospital Chattanooga, part of Lifepoint Health, seeks a Pharmacist to join its care team. You will manage drug therapy protocols, review and interpret orders, provide drug information to staff and patients, and document interventions for quality improvement. Pharmacist degree required; Pharm.D. preferred. PRN schedule with some on-call coverage.

Qualifications

• Graduate of an accredited School of Pharmacy with a Pharmacist degree.
• Pharm.D. degree preferred.

Responsibilities

• Manages drug therapy protocols as part of an interdisciplinary team.
• Reviews, interprets and evaluates medication orders to identify and resolve drug-related problems.
• Provides drug information and education to medical staff, healthcare professionals, and patients.
• Documents interventions and outcomes for drug therapy quality improvement.

How you'll contribute

A Pharmacist who excels in this role:

• Manages drug therapy protocols as a part of an interdisciplinary team.
• Reviews, interprets and evaluates medication orders to identify, prevent and resolve potential or actual drug related problems.
• Provides drug information and education to medical staff, healthcare professionals, and patients, and pharmacy students.
• Documents interventions and their outcomes for use in drug therapy quality improvement efforts.
• Evaluates medication orders for accuracy and completeness prior to order entry. Checks for accuracy after filling by technicians.
