Kindred Hospital Chattanooga, part of Lifepoint Health, is seeking a Pharmacist to join an interdisciplinary team dedicated to improving patient care. You will review medication orders, participate in drug therapy protocols, and document interventions for quality improvement.

Qualifications

• Graduate of an accredited School of Pharmacy with a Pharmacist degree; Pharm.D. preferred.
• Able to work in an interdisciplinary team to optimize drug therapy.
• Experience interpreting and evaluating medication orders and providing drug information.

Responsibilities

• Manages drug therapy protocols as part of an interdisciplinary team.
• Reviews and evaluates medication orders to prevent and resolve drug-related problems.
• Provides drug information to medical staff, healthcare professionals, patients, and students.
• Documents interventions and outcomes for drug therapy quality improvement.
• Ensures accuracy of medication orders before entry and checks after filling by technicians.

How you'll contribute

A Pharmacist who excels in this role:

• Manages drug therapy protocols as a part of an interdisciplinary team.
• Reviews, interprets and evaluates medication orders to identify, prevent and resolve potential or actual drug related problems.
• Provides drug information and education to medical staff, healthcare professionals, and patients, and pharmacy students.
• Documents interventions and their outcomes for use in drug therapy quality improvement efforts.
• Evaluates medication orders for accuracy and completeness prior to order entry. Checks for accuracy after filling by technicians.
