Job summary

TriCities Hospital in Hopewell, Virginia is seeking a Pharmacist PRN to support the medication-use process and patient care across shifts. You will manage storage, preparation and dispensing of medications, supervise technical staff, and collaborate with clinicians to ensure safe, effective pharmacotherapy.

Requirements include a ACPE-accredited PharmD, VA licensure by exam or reciprocity, and experience with pharmacy information systems.

Qualifications

  • Graduate of ACPE-accredited School of Pharmacy.
  • Experience using computer applications, pharmacy information systems.
  • Pharmacist licensed in VA (examination or reciprocity).
  • Pharmacist designation and active licensure required.

Responsibilities

  • Manage storage, preparation and dispensing of medicinal agents; supervise staff involved in these processes.
  • Maintain automated dispensing processes and related systems.
  • Provide education on rational medication therapy to healthcare professionals, patients and the public.
  • Retrieve, analyze and interpret scientific literature to support drug information and therapy decisions.
  • Design, monitor and intervene in pharmacotherapy to ensure safe, effective patient care.
  • Support formulary stewardship and evidence-based therapy recommendations.
  • Contribute to department objectives, policies and procedures; uphold professional standards.
  • Maintain professional practice, time management and attendance; fulfill additional staffing hours.

Job Summary and Qualifications
  1. Manage systems for storage, preparation and dispensing of medicinal agents using appropriate techniques and following the health systems policies; effectively supervise technical personnel who may be involved in these processes. Evaluate medication orders, accurately and safely, compounding medications in acceptable dosage forms, appropriately package and dispense pre-manufactured dosage forms. Issues controlled substances to patient care areas and maintains records as required by law. Collaborates with all individuals involved in the medication use process, when appropriate. Responsible for planning, organizing, networking, completing and improving the work activities through effective management of time, tasks and available personnel.
  2. Maintains knowledge pertaining to facility utilization of automated dispensing processes including the use of automated dispensing cabinets, over wrap packaging, and bar coding within the facility.
  3. Provide education or in-service on rational medication therapy and medication procedures effectively to healthcare professionals, students, patients, and the public when requested.
  4. Retrieve, analyze, evaluate and interpret scientific literature in order to provide accurate, concise, applicable, and timely drug information response to healthcare professionals. Suggest appropriate, cost-effective therapeutic alternatives to medical staff, as needed.
  5. Design, implement, monitor, evaluate, and intervene in patient pharmacotherapy to ensure effective, safe and economical patient care. Apply clinical pharmacokinetic and pharmacodynamic data to design effective, safe and economical therapeutic medication regimens. Detect and report suspected adverse drug reactions and medication errors accurately and in a timely manner.
  6. Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols, and promoting rational medication therapy selection. Recommends evidence-based medication therapy regimens and monitoring plans.
  7. Participates in implementing goals, objectives, and policy and procedures of the pharmacy department and the facility. Effectively supports management. Demonstrates the values and behaviors of the organization.
  8. Effectively manages own professional practice including time and space; professional ethics; continued education; attendance punctuality; and customer focus. Performs other duties as assigned, including fulfilling additional pharmacy staffing hours when necessary.
  9. Performs other duties as assigned or scheduled.
What qualifications you will need:
