Job summary

Froedtert Health in Milwaukee, WI is seeking a hospital pharmacist to manage pharmacotherapy on an assigned patient care unit. You will collaborate with physicians and nursing staff to deliver high-quality and cost-efficient drug therapy, and provide distribution support as needed.

Required is a PharmD or Bachelors in Pharmacy from an ACPE-accredited school, plus Wisconsin license or eligibility within 3 months.

Qualifications

• Pharmacist degree required from ACPE-accredited school.
• Active Wisconsin Pharmacist License or eligible to obtain within 3 months.
• ASHP/PSW memberships preferred.

Job Summary:

Responsible for all aspects of pharmacotherapy on an assigned patient care unit. Provides clinical support and distribution services in coordination with physician and nursing services in delivering quality and cost-effective drug therapy to patients. Other duties as assigned.

Experience Description:

A minimum of one year hospital or three years of other pharmacy experience is preferred.

Education Description:

Bachelor's degree in Pharmacy or Pharm. D. from an ACPE certified school of pharmacy is required. Successful completion of an ASHP accredited PGY1 residency is preferred.

Special Skills Description:

Demonstrates effective use of computers and applications (e.g. MS Word, Excel, Outlook) as appropriate to practice and communicates effectively. Active membership in the Pharmacy Society of Wisconsin (PSW) preferred. Active membership in the American Society of Health System Pharmacists (ASHP) and/or other national pharmacy organizations as approved by the Pharmacy Administration preferred.

Licence Description:

Active Wisconsin Pharmacist License is required or eligible for and must obtain within 3 months.
