Qualifications

• Strong analytical, problem-solving, written communication and interpersonal skills

• Licensed or license-eligible in the State of Texas

Responsibilities

• Ensures safe transitions of care by completing medication reconciliation and final verification of admission and discharge orders for all patients.
• Evaluate and verify WellSky physician's orders for dosing, interactions, and therapeutic appropriateness.
• Clarifies physicians' orders with prescriber or designated practitioner when necessary.
• Demonstrates antimicrobial stewardship and evidence-based regimens.
• Assists with monitoring and appropriate prescribing of anticoagulation therapy.
• Provides pharmacotherapy recommendations and drug information to medical and nursing staff.
• Supervises pharmacy technicians in dispensing, ordering and inventory management.
• Thorough documentation of all clinical interventions.
• Upholds Ernest Health policies and procedures.

Responsibilities

• Ensures safe transitions of care by completing comprehensive medication reconciliation and final verification of admission and discharge orders for all patients.
• Evaluate and verify WellSky physician's orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route and rate of administration and therapeutic redundancy
• Clarifies physicians' orders, when necessary, through direct and timely contact with prescriber or designated practitioner
• Demonstrates knowledge with antimicrobial stewardship ,which includes therapy optimization, de-escalation, and selection of appropriate, evidence-based antimicrobial regimens.
• Assists with monitoring and appropriate prescribing of anticoagulation therapy
• Provides patient-specific pharmacotherapy recommendations and drug information to the medical and nursing staff

• Supervises pharmacy technicians in activities related to medication dispensing, ordering and inventory management.

• Thorough documentation of all clinical interventions
