Job summary

UNC Health in Chapel Hill, NC is seeking a pediatric pharmacist to provide patient-centered care in the Pediatric Emergency Department and support for Children’s Hospital services. The role supports Hematology/Oncology, Bone Marrow Transplant, Pediatric, Cardiac, Neonatal ICU and more, with evening shifts and a rotating schedule to cover seven days per week.

Qualified candidates must have a PharmD or BS Pharmacy Degree and be licensed in NC or eligible within 6 months; PGY1/PGY2 in pediatrics

Qualifications

• Active pharmacist licensure with the NC Board of Pharmacy or eligible to obtain licensure within 6 months of orientation.
• Completion of a PGY1 or PGY2 residency in pediatrics or equivalent experience preferred.
• No prior experience required.

Responsibilities

• Ensures timely medication access for service patients; reviews and verifies prescriptions and clarifies issues with prescribers and staff.
• Analyzes patient data to provide optimal drug therapy recommendations and interventions.
• Performs protocol-driven clinical services including dose monitoring and antimicrobial stewardship.
• Compounds and oversees preparation and dispensing of medications, including sterile products.
• Maintains patient profiles in the EMR to facilitate communication.
• Discharges patients with medication reconciliation and counseling as needed.
• Practices medication safety and participates in Code Blue situations for pediatrics.
• Precepts students and residents and trains new staff as required.

Summary

The University Of North Carolina Hospital Department Of Pharmacy has an immediate opening for a highly motivated individual interested in providing patient care to our pediatric patient population. This pharmacist will work in a multidisciplinary environment to provide comprehensive pharmaceutical care to patients in the Pediatric Emergency Department. The position will also provide support for Children’s Hospital pharmacy services, including Hematology/Oncology, Bone Marrow Transplant, Pediatric, Cardiac, and Neonatal Intensive Care, Pulmonary/Cystic Fibrosis, Endocrinology, and Cardiology. This position will primarily work evening shifts, with a rotating schedule designed to provide Pediatric Emergency Department pharmacist coverage seven days per week in collaboration with another clinical pharmacist.

N.C. Children’s Hospital has 172 inpatient beds and a comprehensive children’s outpatient center located in the same building, enabling patients to see multiple specialists in a single visit.

UNC Children’s Emergency Department offers a full range of primary, tertiary, and quaternary subspecialty care attached to the Triangle’s only Level I pediatric trauma center (one of only 41 in the nation). The Pediatric Services at UNC Hospitals and Clinics are in a period of growth. Our mission for the pediatric pharmacy is to provide quality, patient-centered care, in collaboration with other healthcare professionals, for all pediatric patients within the Children’s Hospital by practicing evidenced-based medicine, optimizing medication use and distribution while ensuring medication safety.

The Department of Pharmacy is a dynamic and critical part of patient care provided by the UNC Health Care system. UNC’s Department of Pharmacy is an organization of more than 500 dedicated employees with focused attention on caring for patients of the UNC Health Care system. The Department is highly interactive with the University of North Carolina Eshelman School of Pharmacy, located on the same Chapel Hill campus. UNC Hospitals and Clinics acts as a major clinical and administrative training site for these pharmacy students, in addition to its greater than 40 pharmacy residents. The position will be responsible for taking students from the UNC Eshelman School of Pharmacy as well as precepting both PGY1 and PGY2 residents in pediatrics.

Qualified candidates will have earned a BS in Pharmacy or a Doctor of Pharmacy degree from an ACPE-accredited school/college of pharmacy and must be licensed or eligible for licensure to practice pharmacy in the State of North Carolina. Completion of a PGY1 or PGY2 residency in pediatrics or an equivalent level of pharmacy experience is also preferred. Candidates should possess and demonstrate excellent initiative, communication, analytical skills, organization, and collaboration.

Responsibilities

• Ensures timely medication access for service patients; including order review and verification. Clarifies incomplete, inaccurate, or contraindicated prescriptions with the prescriber, nurse, or other health professional, as appropriate. Identifies and intervenes on problems related to the medication needs of the patient.
• Utilizes and analyzes patient data to provide optimal drug therapy recommendations. Assess the patient's medication therapy regimen and makes appropriate clinical interventions with the provider.
• Performs protocol-driven clinical services such as therapeutic dose monitoring, antimicrobial stewardship activities, etc.
• Compounds and oversees the preparation and dispensing of medications including sterile products using standard formulas, processes and procedures as required.
• Maintains patient profiles within electronic medical record to facilitate communication
• Complies with the medication formulary, and therapeutic substitution and other programs designed to optimize drug therapy while minimizing cost as required.
• Facilitate discharge and transfer of patients (ie. medication reconciliation and discharge counseling).
• Practices medication safety including adverse drug event and medication error reporting.
• Participates in Code Blue situations for pediatric patients.
• Precepts students, residents and participates in training new pharmacy staff as required.

• Active pharmacist licensure with the NC Board of Pharmacy or obtainable before initiation of employment. Recent graduates of ACPE-accredited colleges/schools of pharmacy may fill this role while pending initial pharmacist licensure, but are subject to limitations mandated by the NC Board of Pharmacy and must obtain licensure within 6 months of orientation or sooner if required for operations.
