Job summary

JPS Healing Wings & Infectious Disease Clinic seeks a Clinical Pharmacist Disease Specialist for outpatient HIV care on a travel assignment in Texas. The role is 40 hours weekly, Monday to Friday, with dayshift hours.

The position requires a PharmD, a 2-year ASHP HIV residency, and at least 5 years of direct HIV patient care. A Texas pharmacy license and BLS are required. Onsite patient visits are part of daily duties.

Qualifications

• Doctorate of Pharmacy or equivalent from an accredited College of Pharmacy.
• 2-year ASHP residency in HIV.
• 5 years of direct HIV patient care demonstrating medication management.
• Pharmacy license from the state board or eligibility for the same.
• BLS certification.

Responsibilities

• Provide clinical leadership for pharmaceutical care in the area of responsibility.
• Serve as a resource for outpatient and inpatient clinicians for HIV and/or Hepatitis pharmacotherapy.
• Collaborate with the inpatient infectious diseases clinical pharmacist to transition HIV patients from the hospital setting to the clinic on appropriate therapy.
• Develop and optimize pharmacotherapeutic regimens through collaborative practice agreements.
• Perform drug utilization reviews, monitor for drug interactions, and potential adverse events.
• Minimize polypharmacy in patients and develop patient medication action plans with self-management goals.
• Act as a preceptor for pharmacy students or residents and participate in education.
• Participate as a key member of the multidisciplinary team in patient care.
