• Texas State Board of Pharmacy license or eligibility.
• BLS certification.

Responsibilities

• Provide clinical leadership in pharmaceutical care within HIV and Hepatitis C programs.
• Engage directly with patients, seeing 14 patients per day as a mid-level provider during the day shift.
• Collaborate with multidisciplinary teams to transition HIV patients from hospital settings to outpatient care.
• Develop and optimize pharmacotherapeutic regimens through collaborative practice agreements, performing drug utilization reviews.
• Serve as a preceptor for pharmacy students and residents.
• Work closely with the Inpatient Infectious Diseases Clinical Pharmacist to manage medication therapy and minimize polypharmacy.
