Job summary

ECU Health Pharmacy is seeking a licensed pharmacist to join our team at ECU Health Medical Center in North Carolina. You will review medications, ensure appropriate therapy, and collaborate with nursing and physicians to optimize patient outcomes.

The role includes medication histories, reconciliation, discharge counseling, and reporting adverse events. Requires NC license and BLS within 30 days; hospital experience preferred.

  • Maintain active NC pharmacy license and provide proof of renewal prior to December 31 annually.
  • American Heart Association BLS required within 30 days of hire.

Responsibilities

  • Review patient medication profile before verifying orders and evaluate orders for indication, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
  • Ensure appropriateness of therapy by reviewing patient history and demographics, recommending formulary alternatives, developing drug monitoring plans, interpreting laboratory data, monitoring and documenting pharmacokinetics for targeted drugs.
  • Demonstrate competency in applying drug dosing guidelines, clinical lab data and pharmacokinetic properties to individualize drug therapy.
  • Communicate drug therapy problems directly with nursing and/or prescriber and document interventions.
  • Provide care appropriate to patient age(s).
  • Perform medication histories, medication reconciliation and discharge counseling as required.
  • Provide drug information to health professionals and to patients.
  • Detect, monitor, document and report adverse drug reactions and medication errors.
  • Maintain professional relationships, supervise pharmacy technicians, dispense investigational drugs per protocols, and attend department education activities.

Position Summary

Pharmacists at ECU Health are committed to the safe and effective use of medications through maximizing drug distribution and optimizing drug therapy outcomes; educating patients, providers, and ourselves on best practices; and fully integrating pharmacists into the continuum of care. Pharmacists report directly to a designated Pharmacy Supervisor based on area of practice.

Responsibilities
  • Review patient medication profile before verifying orders and evaluate orders for indication, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions.
  • Ensure appropriateness of therapy by reviewing patient history and demographics, recommending formulary alternatives, developing drug monitoring plans, interpreting laboratory data, monitoring and documenting pharmacokinetics for targeted drugs, and may include credentialing as a Clinical Pharmacist Practitioner.
  • Demonstrate competency in applying drug dosing guidelines, clinical lab data and pharmacokinetic properties to individualize drug therapy.
  • Communicate drug therapy problems directly with nursing and/or prescriber and document all interventions.
  • Provide care appropriate to patient age(s).
  • Perform medication histories, medication reconciliation and discharge counseling as required.
  • Provide drug information to health professionals and to patients.
  • Detect, monitor, document and report adverse drug reactions and medication errors.
  • Maintain professional relationships, handle communications professionally, supervise pharmacy technicians, dispense investigational drugs per protocols, and attend department education activities.
  • Completion required: obtain continuing education credits, complete annual competency assessments, meet TJC requirements and occupational health screening, comply with state and federal regulations, follow hospital and departmental policy, maintain confidentiality, and comply with HIPAA and HITECH regulations.

Minimum Requirements
  • B.S. or PharmD in pharmacy
  • 0 years of relevant pharmacy experience (post-graduate)
  • Licensed as a pharmacist by the NC Board of Pharmacy by start date.
  • Maintain active NC pharmacy license and provide proof of renewal prior to December 31 annually.
  • American Heart Association BLS required within 30 days of hire.
Preferred Requirements
  • Hospital experience preferred.
