Job summary

AIDS Healthcare Foundation seeks a Clinical Pharmacist 1 Multisite to expand primary care functions across multiple sites, providing vaccines, PrEP/PEP therapies, diabetes monitoring, and smoking cessation programs.

You will collaborate with AHF providers to implement best practices, ensure policy consistency, and support patient-centered care across locations while adapting to a dynamic healthcare environment.

Qualifications

• Doctor of Pharmacy (PharmD) required or equivalent.
• CA licensure or eligibility to obtain license.
• Immunization certification and provider status.
• Ability to work across multiple sites with collaboration.

Responsibilities

• Administer vaccines and provide preventive care in collaboration with AHFs team.
• Manage pharmacotherapy and SME programs with prescriptive authority where allowed.
• Promote health, disease prevention, and adherence across multisite locations.

Job description

A Clinical Pharmacist 1 Multisite contributes to AHF by expanding primary care functions of pharmacists at more than one site to provide individualized health promotion and disease prevention, including administration of vaccines, long-acting HIV treatments, diabetes monitoring, nicotine reduction programs and other subject matters, in collaboration with other AHF providers. A Clinical Pharmacist – Multisite enhances the reputation of AHF by providing valuable contributions to more than one location by bringing “best practice” and collaborative ideas, as well as providing consistent application of processes, procedures and policy throughout the organization. You will be a passionate advocate for our top initiatives. The Clinical Pharmacist 1 Multisite will work as a part of a close-knit team to provide individualized health promotion and disease prevention, including administration of immunizations, diabetes and smoking cessation programs, as well as other Subject Matter Expertise (SME) programs as developed by AHF, with appropriate collaborative drug therapy management authority. This includes prescriptive authority where this is legally and organizationally authorized such as PrEP and PEP therapies
