Job summary

AIDS Healthcare Foundation is seeking a Clinical Pharmacist - Multisite to extend primary care functions across multiple sites, providing immunizations, diabetes monitoring and HIV therapies in collaboration with AHF teams.

The role emphasizes patient-centered care, adherence to AHF core values, and prescriptive authority where allowed (PrEP/PEP) within a collaborative drug therapy management framework. You will work with a close-knit team to advance programs and patient outcomes.

Qualifications

• State Pharmacist License is required.
• Experience with immunizations and SME programs is preferred.
• Ability to work across multiple sites and collaborate with a multidisciplinary team.

Responsibilities

• Provide health promotion and disease prevention across multiple sites.
• Administer vaccines and immunizations as part of patient care.
• Support diabetes monitoring and smoking cessation programs.
• Collaborate with AHF providers and manage prescriptive authority where legally allowed (e.g., PrEP/PEP) within a CMT framework.
