Job summary

AIDS Healthcare Foundation seeks a Clinical Pharmacist – SRT to join a multi-location team across Dallas, TX. You will provide individualized health promotion, disease prevention, and immunization services, while supporting diabetes monitoring and smoking cessation programs.

In collaboration with AHF Providers, you will engage in SME programs and, where legally allowed, prescriptive authority to improve patient outcomes within the AHF Healthcare System.

Qualifications

• Licensed pharmacist in the respective state.
• Experience with clinical pharmacy services including immunizations.
• Ability to participate in collaborative drug therapy management.

Responsibilities

• Provide individualized health promotion and disease prevention.
• Administer immunizations and manage diabetes monitoring programs.
• Lead smoking cessation programs and other SME initiatives.
• Operate with prescriptive authority where legally authorized, including PrEP/PEP as applicable.

Job description

The Clinical Pharmacist – SRT contributes to AHF by being assigned to more than one location, based on scheduling/geographical needs. Expanding primary functions of pharmacist such as providing individualized health promotion & disease prevention, including vaccine, long-acting HIV medicine administration, Diabetes monitoring, Nicotine reduction programs and other subject matters, in collaboration with other AHF Providers.

The Clinical Pharmacist – SRT enhances the reputation of AHF by improving the outcomes of AHF patients, enhancing current AHF services, expanding opportunities for professional growth and satisfaction, and assisting with ongoing development of the AHF Healthcare System. You will be a passionate advocate for our top initiatives.

The Clinical Pharmacist – SRT will work as a part of a close-knit team to provide individualized health promotion and disease prevention, including the administration of immunization, Diabetes and Smoking Cessation programs, as well as other subject matter expertise (SME) programs developed by AHF, with appropriate collaborative drug therapy management authority. Includes prescriptive authority where this is legally & organizationally authorized, such as PrEP and PEP therapies.
