Job summary

• Conduct comprehensive medication reviews (CMRs) and targeted medication reviews (TMRs).
• Assess regimens for appropriate selection, dose, route, frequency, indication, effectiveness, safety, and adherence.
• Identify, prevent, and resolve medication-related problems.
• Develop individualized medication action plans.
• Communicate recommendations to providers and healthcare teams.
• Follow up on medication changes and clinical outcomes.

Responsibilities

• Clinical Pharmacist Assessment: Evaluate patient-specific factors including drug-disease interactions, allergies, renal and hepatic function, and labs.
• Assess contraindications, duplicate therapy, and monitoring needs.
• Identify affordability and access barriers to medications.
• Patient Care & Education: Provide medication counseling and education to patients and caregivers.
• Promote adherence and self-management strategies.
• Educate patients regarding medication purpose, benefits, adverse effects, monitoring, and lifestyle changes.
• Support patients in understanding and managing chronic conditions.
• Chronic Disease Management Support: Provide pharmacist-led interventions and monitoring for diabetes, anticoagulation, asthma/COPD, and other chronic diseases.

Patient Care & Education
