Responsibilities

The Pharmacist provides advanced medication therapy management (MTM) services within referral-based Hospital Outpatient Department (HOPD) pharmacist-managed clinics. Practicing under a collaborative practice agreement (CPA) and in accordance with the California State Board of Pharmacy, the pharmacist manages complex and chronic conditions by initiating, adjusting, and discontinuing medications; conducting patient assessments; and monitoring therapeutic outcomes. The role includes collaborating with prescribers under a CPA, coordinating medication access, promoting adherence through patient education, documenting care in the electronic health record (EHR), and supporting quality improvement and professional development initiatives. • Provides comprehensive medication management services within HOPD referral-based pharmacist-managed clinics.
• Initiates, modifies, and discontinues medication therapy under a CPA and in accordance with state regulations.
• Conducts comprehensive patient assessments, including medication reconciliation and review of medical history under a CPA.
• Orders, interprets, and monitors laboratory tests to ensure medication efficacy and safety under a CPA.
• Manages complex and high-risk chronic disease states (such as oncology supportive care, weight loss, diabetes, hypertension, heart failure, anticoagulation, COPD and hyperlipidemia) under a CPA.
• Develops and implements individualized, evidence-based treatment plans aligned with clinical guidelines under a CPA.
• Monitors therapeutic outcomes and adjust pharmacotherapy to achieve optimal clinical goals under a CPA while identifying and resolving medication-related problems.
• Provides patient education and counseling to improve medication adherence and support disease self-management through in-person or virtual visits.
• Collaborates with referring providers, interdisciplinary teams, payers, retail pharmacies, and specialty pharmacies to coordinate care and address medication access issues.
• Documents all patient encounters and clinical interventions in the electronic health record (EHR) and track clinical outcomes and quality metrics to support value-based care and quality improvement initiatives.
• Serves as a preceptor for pharmacy students and residents when applicable.
• Performs other job-related duties as assigned.

Knowledge, Skills, and Abilities

• Strong pharmacotherapy knowledge for chronic and complex conditions.
• Understanding of state pharmacy regulations, scope of practice, and collaborative practice agreements.
• Familiarity with evidence-based guidelines, lab monitoring, and medication therapy management.
• Ability to assess patients, review medication regimens, and participate in development of individualized treatment plans under a CPA.
• Supports medication initiation, modification, and discontinuation per collaborative agreement protocols.
• Patient education and counseling, including virtual care delivery.
• Proficiency with Epic or similar EHR systems (e.g., Epic) for documentation and tracking clinical outcomes.
• Strong communication, problem-solving, and interdisciplinary collaboration skills.
• Ability to adapt care delivery to diverse patient populations and telehealth environments.
• Commitment to patient-centered care and quality improvement initiatives.

Qualifications

• Doctor of Pharmacy (PharmD) Degree from an ACPE-accredited institution required.
• PGY-1 Pharmacy Practice Residency or significant ambulatory clinical experience required.
• PGY-2 residency in ambulatory care or related specialty preferred.
• Advanced clinical expertise in chronic disease management preferred.
• Experience or familiarity with 340B program operations and compliance preferred.
• Demonstrated pharmacy practice experience in ambulatory or HOPD pharmacist-managed clinics preferred.

Licenses and Certification

• Current and unrestricted Registered Pharmacist (RPh) License in California required.
• Board certification (BCPS, BCACP, or equivalent) required within 18 months of hire.
• Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required.
