Job summary

Pharmacy professionals are sought to provide comprehensive inpatient and outpatient services at the Phoenix VA Health Care System in Phoenix, Arizona.

Selected candidates will support pharmacy operations and deliver services to Veterans in accordance with VA standards and applicable regulations. Expect rotating shifts including days, evenings, nights, weekends, and holidays in a hospital-based setting.

Qualifications

• ACPE-accredited pharmacy degree required.
• Valid pharmacist license in any US state.
• Eligible for VA credentialing and privileging.
• Minimum 24 months of relevant pharmacy experience in the last 36 months.
• Ability to read, write, and speak English fluently.
• Ability to work independently and within a healthcare team.
• Willingness to work rotating shifts, including nights, weekends, and holidays.

Responsibilities

• Provide inpatient and outpatient pharmacy services.
• Review, interpret, and dispense medications accurately.
• Ensure safe and effective medication management.
• Collaborate with physicians, nurses, and other healthcare professionals.
• Support clinical pharmacy operations and patient care.
• Maintain accurate pharmacy records and documentation.
• Comply with VA policies and regulations.
• Report patient outcomes and support quality improvement activities.
• Work day, evening, overnight, weekend, and holiday shifts as required.
