Job summary

The VA Health Care System seeks experienced Pharmacists to support inpatient and outpatient pharmacy services for Veterans. Provide medication safety, patient consultations, and collaborate with care teams to optimize patient outcomes.

Requirements include active licensure in any U.S. state, ACPE-accredited BS in Pharmacy or Pharm.D., and at least 3 years of inpatient/outpatient experience (VA preferred). Compliance with VA policies and teamwork are essential.

Qualifications

• Active, unrestricted Pharmacist License in any U.S. State.
• ACPE-accredited degree: BS in Pharmacy or Pharm.D.
• Minimum 3 years of inpatient or outpatient pharmacy experience (VA experience preferred).

Responsibilities

• Verify and dispense medications for inpatient and outpatient use.
• Conduct medication therapy management and provide patient counseling.
• Manage controlled substances and ensure compliance with VA regulations.
• Collaborate with healthcare providers to develop and adjust medication plans.
• Oversee medication preparation, compounding, and administration protocols.
• Document all pharmaceutical activities in VISTA/CPRS EMR.
• Provide drug information and training to pharmacy staff and healthcare teams.
• Maintain compliance with VA policies, state and federal laws, and pharmacy best practices.
