Job summary

Children's hospital seeks a pharmacist to ensure safe and accurate medication use. You will evaluate orders, supervise staff, and answer drug-related questions for professionals and the public. Counseling patients and families is a key part of the role.

Responsibilities include pharmacokinetic monitoring, adverse event reporting, and participating in medication reconciliation during patient transitions. Candidates should have a PharmD or PharmD-equivalent and be licensed; BLS is required, BCPS

Qualifications

  • Doctor of Pharmacy degree or equivalent required; state license required.
  • Bachelor’s degree in Pharmacy from an accredited college or university required.
  • PGY1 residency or minimum 3 years’ hospital pharmacy experience preferred.
  • Licensed pharmacist (PHARMD) and BLS certification required; BCPS preferred.

  • Supervise supportive pharmacy personnel and ensure accurate dispensing.

Schedule: Full time, variable


10 - 8 hour shifts every 2 weeks within hours of 0630-2130 Sun-Sat


Average 1 evening (1230-2100) per week. Cover 1 weekend per month. On call 2 weekends per year.

A Brief Overview

Ensures safe, accurate and appropriate use of medications, by evaluating prescribers\' orders for dosing, allergies and contraindications. Supervises the work of supportive pharmacy personnel. Answers pharmacy-related questions for other health professionals and the general public. Performs pharmacokinetic monitoring and consultations, reports adverse drug events, interventions (iVents), and variances involving medications. Recommends appropriate and cost-effective drug therapy options. Provides medication counseling to patients and their families. Actively participates and facilitates the medication reconciliation process upon admission, transfer, and discharge of patients. Promotes and participates in the defined missions of the hospital and the pharmacy. Actively promotes the profession of pharmacy by participating in the education of students on clerkships and experiential experiences, as well as pharmacy residents.


Essential Functions


  • Appropriately reviews medication therapy and intervenes with healthcare team as warranted. Evaluates medication order for appropriate medication selection, dosage, route, frequency, duration of therapy and drug interactions (drug‑drug, allergy, drug‑disease, drug‑lab, drug‑food) in relation to the patient\'s clinical status. Performs daily profile review to assess appropriateness of medication therapy in relation to patient\'s ongoing and changing disease state and clinical status. Appropriately and routinely utilizes formulary agents; follows appropriate procedures for the procurement of non‑formulary items and the use of patient\'s own medications. Monitors pharmacokinetic patient profiles in accordance with policy. Performs accurate and timely pharmacokinetic consults. Demonstrates competency verifying and or entering orders into the electronic medical record.

  • Responsible for the timely and accurate dispensing of pharmaceuticals, ensuring proper preparation, labeling, dispensing and charging. Reviews medication orders and appropriately triages urgent, routine, and PRN medication orders. Understands and demonstrates appropriate order entry, preparation and documentation of the following pharmaceutical products: controlled substances, chemotherapy, TPN solution, injectables, oral preparations, respiratory care items, home medications, non‑formulary medications, investigational medications, etc. Provides appropriate supervision and timely checking of pharmaceutical products prepared by supportive pharmacy personnel. Proficient in describing appropriate delivery of all medications including routine meds, controlled substances, and chemotherapy. Effectively identifies and resolves routine professional practice problems on a regular basis using critical evaluation skills.

  • Appropriately manages technology in the day‑to‑day delivery of pharmaceutical care. CADD® pump programming. BAXA® compounder setup and trouble shooting. ABACUS® order entry. Basic PYXIS® functions. Electronic variance reporting system. Intervention documentation (iVents). Retrieval of Policy and Procedures from the Intranet. Electronic drug information resources (Lexi‑comp, Micromedex, etc.).

  • Provides information to healthcare professionals and families regarding drug therapy. Answers drug related questions in a timely and professional manner and utilizes other resources as necessary. Provides discharge teaching instructions, including patient information sheets and medication calendars if warranted, and documents in EMR appropriately. Participates in educations opportunities with healthcare practitioners, including students and residents.

  • Based on departmental/organizational need may be required to participate in Transplant program functions.

  • Regular attendance at work is an essential function of the job.

Education Qualifications


  • Doctor of Pharmacy Required or

  • Bachelor\'s Degree of Science from an accredited college or university in Pharmacy Required


Experience Qualifications


  • PGY1 (Post Graduate Year 1 Residency) or minimum 3 years\' experience in a hospital pharmacy setting preferred. Preferred


Skills and Abilities


  • Mathematical skills necessary for algebraic conversion, subtractions, multiplication, and division.

  • Verbal and written communications skills sufficient/necessary to receive and deliver information accurately among pharmacy staff and others.


Licenses and Certifications


  • PHARMD - Licensed Pharmacist Required and

  • BLS - Basic Life Support through the American Heart Association Required and

  • BCPS - Board Certified Pharmacotherapy Specialist Preferred and
