Qualifications

• PharmD degree or equivalent.

Responsibilities

• Participating in multidisciplinary inpatient rounds and care transitions planning for high-risk patients.
• Performing comprehensive admission and discharge medication reconciliation.
• Identifying, resolving, and documenting medication-related problems.
• Providing discharge medication counseling and patient education utilizing teach-back methodologies.
• Conducting post-discharge follow-up calls and clinical assessments for adherence barriers and care coordination needs.
• Facilitating medication access, prior authorizations, appeals, and discharge prescription coordination.

Education

Doctor of Pharmacy degree (Pharm D) or equivalent training
