Job summary

A leading women's healthcare facility in Baton Rouge seeks an experienced Pharmacist responsible for preparing and dispensing medications.   • Calculates and verifies doses for high-risk medications as needed.
  • Compounds low and medium risk sterile preparations.
  • Monitors drug therapy for contraindications and interactions.
  • Records use of controlled substances as per policy.
  • Verifies drugs’ contents and expiration dates in various trays.
  • Any other duties as assigned.

Job description

The Pharmacist prepares and dispenses pharmaceuticals as ordered by qualified prescriber; approves the drug usage, strength, dosage regimen and route of administration. Performs other duties assigned by department management according to policies and procedures and the mission of Woman's Hospital.

Requirements:

  • Current licensure from the Louisiana Board of Pharmacy.
  • Experience in hospital pharmacy and IV preparation is preferred.

Responsibilities:

  • Documents and maintains patient profile accurately to include information on allergies, disease history, food/drug interactions, patient education, etc.
  • Interprets medication orders and provides pharmaceutical care in accordance to patient's risk level. Consults appropriate references when needed.
  • Prepares and dispenses medications using appropriate techniques and following the hospital's policies and procedures.
  • Performs prescription counseling for patients or caretakers on discharge medications as requested.
  • Calculates and verifies doses for neonatal drugs, chemotherapy, TPNs, aminoglycosides and Vancomycin upon request, and other high risk medications as needed.
  • Compounds low and medium risk sterile preparations including epidural bags and syringes, large volume parenteral, TPNS, IV syringes for neonates, chemotherapy, anesthesia unit doses, etc.
  • Monitors drug therapy for contraindications, interactions, adverse drug reactions, therapeutic duplication and appropriateness.
  • Records the use of controlled substances according to policy and procedure. Verifies controlled substance count of the narcotic being dispensed. Monitors controlled substance dispensing and receipt records for accuracy. Reports discrepancies promptly to director.
  • Verifies contents and expiration dates of drugs in anesthesia trays, transport bags, crash cart trays, neonatal resuscitation trays, and other exchange systems in a timely manner.
  • Any other duties as assigned by Woman's Hospital.

Monday - Friday

Every third/fourth weekend

Pay Range:

Hourly/Exempt
