Job summary

Valley Presbyterian Hospital is seeking a pharmacist to provide comprehensive pharmaceutical care through clinical oversight, medication management, and collaboration with the healthcare team.

You will review orders, monitor therapy, interpret labs, educate patients and staff, supervise technicians, ensure accurate dispensing, and participate in safety and compliance initiatives in a team-focused environment.

Qualifications

  • Must possess and maintain a California Pharmacist license at hire date.
  • Ability to obtain LA City Fire Card within 30 days.
  • PharmD degree preferred or equivalent clinical experience.

Responsibilities

  • Review medication orders for accuracy, appropriateness, and safety.
  • Monitor drug therapy including dosing, interactions, and outcomes.
  • Provide clinical interventions and collaborate with physicians on plans.
  • Interpret lab values and assess patient medication profiles.
  • Educate patients and healthcare staff on medication therapy.
  • Supervise pharmacy technicians and support staff.
  • Ensure accurate and timely dispensing of medications and IVs.
  • Monitor for adverse drug reactions and medication errors.
  • Support formulary management and non-formulary drug procurement.
  • Participate in performance improvement, safety, and compliance initiatives.

At Valley Presbyterian Hospital, our pharmacy team plays a critical role in patient safety and clinical outcomes. We're seeking a skilled pharmacist to provide comprehensive pharmaceutical care through clinical oversight, medication management, and collaboration with the healthcare team.

What You'll Do
  • Review medication orders for accuracy, appropriateness, and safety
  • Monitor drug therapy including dosing, interactions, and therapeutic outcomes
  • Provide clinical interventions and collaborate with physicians on medication plans
  • Interpret lab values and assess patient medication profiles
  • Educate patients and healthcare staff on medication therapy
  • Supervise pharmacy technicians and support staff
  • Ensure accurate and timely dispensing of medications and IV preparations
  • Monitor for and report adverse drug reactions and medication errors
  • Support formulary management and non-formulary drug procurement
  • Participate in performance improvement, safety, and compliance initiatives
What You Bring

Requirements
  • Must possess and maintain a current Pharmacist license issued by the State of California Board of Pharmacy upon hire/start date.
  • Ability to obtain LA City Fire Card within 30 days
Preferred
  • Doctor of Pharmacy (PharmD) degree preferred
  • ASHP-accredited residency or equivalent clinical experience
  • Experience with Meditech or similar systems
