Job summary

Henry Mayo Newhall Hospital in Valencia, CA is seeking a Pharmacist to provide pharmaceutical services to patients in accordance with Title 22, OBRA, and ASHP guidelines.

You will compound, dispense, and provide consultative services including vancomycin, anticoagulation, and total parenteral nutrition management, while supporting antibiotic stewardship and interdepartmental rounds. Involvement in precepting students and residents is expected.

Qualifications

  • Current CA Registered Pharmacist license
  • Graduate of an ACPE accredited School of Pharmacy or equivalent
  • Pharm.D. preferred
  • PGY1 residency or 1-3 years inpatient experience preferred

Responsibilities

  • Provide pharmaceutical services to patients in accordance with regulatory requirements.
  • Compounding and dispensing medications and providing consultative services (vancomycin, anticoagulation, TPN management).
  • Engage in antibiotic stewardship and participate in code response.
  • Participate in multidisciplinary rounds and precept students and residents.
  • Assist planning and execution of pharmacy operations and regulatory compliance.

Job Summary:

The Pharmacist is responsible and accountable for providing pharmaceutical services to the patient in accordance with regulatory requirements (Title 22, OBRA), professional standards (ASHP guidelines), and state and federal laws.These services involve compounding and dispensing drugs; providing consultative services including vancomycin, anticoagulation, and total parenteral nutrition management; antibiotic stewardship; code response;multi-disciplinary rounds; precepting of students and residents; assisting in the planning, coordination, and execution of pharmacy operations and programs for the enforcement of state and federal laws and regulations pertaining to drugs.Works under minimal supervision with extensive latitude for the use of initiative and independent judgment.


What You'll Bring to the Team:


  • Ability to perform Pharmaceutical Care needs of: Neonatal; Pediatric; Adult; and Geriatric patients.

  • Ability to perform pharmacokinetic calculations.

  • Ability to determine nutrition needs of patients and formulate a suitable Total Parenteral Nutrition plan to meet those needs.

  • Strong verbal and written communication skills.

  • Effective two way communication skills (listening and speaking); strong interpersonal skills with staff, physicians, and patients.

  • Strong critical reasoning skills in clinical decision-making, planning and organizing, initiative, independent decision-making, problem solving, and ability to interpret data.


Licensure and Certification:


  • Current CA Registered Pharmacist license


Education:


  • Graduate of an ACPE accredited School of Pharmacy or equivalent.

  • Pharm.D. preferred.

  • PGY1 pharmacy residency or 1-3 years inpatient experience preferred


Experience:


  • Health-system experience preferred
