Job summary

Aiken Regional Medical Centers, a 273-bed acute care facility in Aiken, South Carolina, is recruiting a pharmacist with expertise in clinical pharmacy to serve as a resource for pharmaceutical care in the acute care setting.

The role involves interpreting orders, monitoring therapy, and providing clinical consultation to medical staff while supervising pharmacy technicians and ensuring safety and cost-effectiveness in therapy choices.

Qualifications

• Bachelor of Science in Pharmacy required; Pharm.D. preferred.
• 1–3 years hospital experience preferred.
• Current licensed pharmacist in South Carolina.
• Pyxis experience with Pyxis reports preferred.

Responsibilities

• Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately.
• Maintains awareness of developments in pharmacy and health care that relate to job responsibilities and integrates them into their practice.
• Supports the pharmacy's pharma-coeconomic efforts by reviewing targeted medications for compliance with current programs.
• Monitors drug therapy regimens for contraindications, interactions, allergies, adverse reactions and appropriateness.
• Provides clinical consultation and clarifies to practitioners; suggests cost-effective alternatives as needed.
• Supervises pharmacy staff and validates daily activities of technicians, including IV and chemotherapy preparation.

Position Description

Responsible for demonstrating an expertise in clinical pharmacy practice and is able to serve as a resource in the provision of pharmaceutical care to patients in the acute care setting.

Duties

• Interprets medication orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete pharmacy patient medication profiles.
• Maintains awareness of developments in pharmacy and health care that relate to job responsibilities and integrates them into their practice.
• Supports the pharmacy's pharma-coeconomic efforts. Reviews the use of targeted medications for compliance with current pharma-coeconomic programs.
• Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, adverse reactions and appropriateness of drug, dosage form, regimen route, and therapeutic duplications.
• Assists with pharmacokinetic consult service and drug dosing regimens per hospital protocol.
• Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternative to medical staff as needed.
• Supervises and directs pharmacy support personnel. Verifies the daily activities of pharmacy technicians. Verifies the accuracy of all orders, IV admixtures and chemotherapy prepared by the technician.
• Performs other duties as assigned/required.
