Job summary

Family Health Source in Deltona, FL, seeks a Pharmacist to join a community-focused team. You will review prescriptions, counsel patients, and contribute to safe drug therapy within a collaborative care model.

A Florida license and at least one year of pharmacy experience are required; 340B exposure is a plus. You will work with providers and clinical staff to ensure continuity of care, supervise staff, and support workflow in a federally qualified health center.

Qualifications

• Pharmacist degree (BSPharm/PharmD) from an accredited program.
• Current Florida Pharmacist license in good standing.
• At least 1 year of retail or ambulatory care pharmacy experience.
• FQHC/340B experience preferred.
• Experience with Rx30 and eCW, MS Office.

Responsibilities

• Review, interpret, and evaluate prescriptions for accuracy and safety.
• Verify and dispense medications per regulations and policies.
• Review patient histories to optimize drug therapy.
• Provide counseling and education to patients and providers.
• Document pharmacist interventions and support MTM services.
• Collaborate with providers to ensure continuity of care.
• Supervise pharmacy staff and support workflow efficiency.
• Maintain accurate records and comply with HIPAA and safety rules.
• Assist with audits and third-party claim resolutions.
