Clinch Valley Health, a Lifepoint Health facility in Richlands, VA, seeks a Pharmacist to join our team. You will coordinate patient pharmaceutical care, participate in interdisciplinary drug therapy, and review orders for safety and accuracy.

You will also provide drug information and educate staff and patients, documenting outcomes for quality improvement. Requirements include a Bachelor’s degree in Pharmacy and registration with the state board of pharmacy.

• Coordinates and provides the pharmaceutical care of patients.
• Manages drug therapy protocols as part of an interdisciplinary team
• Reviews, interprets and evaluates medication orders to identify, prevent and resolve potential or actual drug related problems.
• Provides drug information and education to medical staff, healthcare professionals, and patients, and pharmacy students.
• Documents interventions and their outcomes for drug therapy quality improvement
• Evaluates medication orders for accuracy and completeness prior to order entry; checks accuracy after filling by technicians.
