Tallahassee Memorial HealthCare seeks a Hospital Pharmacist to join the Emergency Center and related areas, delivering comprehensive clinical pharmacy services and contributing to patient safety and pharmacotherapy optimization.

The role emphasizes diversity, integrity and excellence, with opportunities to train Pharm.D. students and residents, and participate in medication-use programs while supporting safe transitions of care within the EC and inpatient settings.

Qualificações

• Pharm.D. degree from ACPE-accredited pharmacy school.
• Two years of hospital pharmacist experience; PGY-1 may substitute one year.
• Preferred: PGY-1 residency, three years of specialty experience, advanced training (residency/fellowship/board certification).
• Licensure as pharmacist in Florida; eligible per Florida statutes; PALS and ACLS within six months post-hire.

Qualifications

Required Education:

• Doctor of Pharmacy (Pharm.D.) degree from an Accreditation Council for Pharmacy Education (ACPE) accredited pharmacy school.

Required Experience:

• Two (2) years of relevant hospital pharmacist work experience. Human Resources (HR) may accept Postgraduate Year One (PGY-1) residency completion in lieu of one (1) year of hospital pharmacist work experience.

Preferred Qualifications:

• PGY-1 residency completion. Three (3) years of specialty experience. Advanced training (e.g., residency completion, fellowship completion, and/or board certification).

Required Certification/License/Registry:

• Licensed [or eligible for licensure (i.e., meets all statutory requirements for Florida licensure application)] as a pharmacist per Florida Statutes (FS), Title XXXII (Regulations of Professions and Occupations), Chapter 465 (Pharmacy), Sections 007 through 013. Pediatric Advanced Life Support (PALS) and Advanced Cardiovascular Life Support (ACLS) certifications within six (6) months post-hire.

Responsibilities

Job Summary:

• Provides comprehensive clinical pharmacy services (including pharmacokinetic and therapeutic consultations) for the Emergency Center (EC) and associated areas (e.g., Pediatric (department 6050), Trauma Center (department 6234), and Urgent Care Center department 6236)).
• Demonstrates an understanding of diversity.

• Enhances and contributes to a safe environment for all colleagues, visitors, and patients.
• Improves medication use safety in the EC.
• Develops medication use management programs.
• Supplies didactic and experiential clinical pharmacy training to Pharm.D. students and pharmacy residents.
• Facilitates proper medication use information transfers for patients converted from the EC to the inpatient registry.
• Participates in professional development and clinical staff competency.
• Strives to reduce EC associated medication use costs.
• Reviews medication orders prior to administering.
• Performs other duties.
