Job summary

Mason Health is seeking a Pharmacist to provide comprehensive pharmaceutical care for inpatients and outpatients. The role includes medication order review, sterile compounding, and collaboration with clinicians to ensure optimal patient outcomes.

Responsibilities include supervising technicians, recording drug monitoring decisions, and ensuring compliance with state and federal regulations. A Pharm.D. or comparable hospital experience plus WA licensure are required.

Qualifications

  • Required: Pharm.D. or 5 years hospital/Residency experience.
  • Current license as a registered Pharmacist in the State of Washington.

Responsibilities

  • Analyze inpatient and outpatient medication orders for completeness and appropriateness per the 6 rights.
  • Supervise pharmacy technicians and provide support as needed.
  • Distribute medications for inpatients and outpatients, including sterile compounding, bar code scanning, and automated dispensing devices.
  • Analyze lab values for pharmacokinetic dosing and therapeutic drug monitoring.
  • Document interventions and monitoring in the EMR.
  • Gather medication histories for reconciliation and dispense medications per regulations.
  • Respond to codes and coordinate nutrition support as needed.
  • Participate in QA and Drug Use Evaluation programs.

This 0.0FTE/Per-Diem position is scheduled to work 10 HR variable shifts as needed and is UFCW represented.

Compensation:

Non-exempt, $70.71/HR-$115.25/HR

Job Summary:

The pharmacist is responsible for ensuring appropriate pharmaceutical care for patients of Mason Health.

Job Specific Duties & Responsibilities:
  1. Analyze inpatient and outpatient medication orders for completeness and appropriateness as it relates to the 6 rights: person, medication, dose, route, schedule, and documentation.
  2. Supervise the work of the pharmacy technicians and provide support help as needed.
  3. Ensure appropriate distribution of medications for both inpatients and outpatients. This includes but is not limited to sterile compounding, bar code scanning, and automated dispensing devices.
  4. Analyze lab values to perform pharmacokinetic dosing, renal dosing, and therapeutic drug monitoring according to established protocols and current guidelines.
  5. Document therapeutic drug monitoring decisions and clinical interventions into the electronic medical record.
  6. Ensure appropriate medication histories are gathered for medication reconciliation.
  7. Dispense medications and maintain appropriate records according to state and federal regulations.
  8. Answer questions and provide information to clinicians and patients regarding drug interactions, side effects, dosages, and storage of pharmaceuticals.
  9. Respond to emergency response codes in the hospital and maintain ACLS certification.
  10. Provide assistance to other healthcare practitioners in coordinating nutrition support (including total parenteral nutrition).
  11. Assists with development of drug protocols and standing orders.
  12. Participates in Quality Assurance and Drug Use Evaluation programs and completes drug utilization reviews for the Pharmacy and Therapeutics Committee.
  13. Other duties as assigned.
Required Education and Experience:
  1. Doctorate in Pharmacy (Pharm.D.) preferred or 5 years' experience in a hospital or residency.
Required Licenses, Certifications and/or Registrations:
  1. Current license as a registered Pharmacist in the State of Washington.
