Job summary

Reston Hospital Center, part of HCA Healthcare, is seeking a Pharmacist to join our team in Reston, VA. The role involves reviewing orders, counseling patients, and ensuring safe medication use across a busy hospital setting.

Requirements include a PharmD or BS from an ACPE-accredited program, Virginia Pharmacist license, and BLS. Hospital experience preferred; competitive benefits offered.

Qualifications

• Graduate of a college of pharmacy with BS or Pharm. D accredited by ACPE.
• Licensed Pharmacist in the Commonwealth of Virginia
• BLS Certification
• Previous hospital pharmacist experience preferred

Responsibilities

• Assesses physician orders for accuracy, allergies, interactions and appropriate dose.
• Documents interventions and communicates with health care workers.
• Counsels patients on medication use at discharge for pediatrics and other patients on multiple meds.
• Monitors drugs with narrow therapeutic range or high use and makes recommendations to optimize outcomes.
• Reviews antibiotic therapy with culture/sensitivity considerations and cost-effective alternatives.
• Ensures proper storage and handling of medications and completes unit inspections monthly.
• Assists in teaching programs for nursing and health care staff.
• Provides drug information to patients, nurses, physicians and others.
• Directs technicians in IV admixture, ICU meds, TPN, chemotherapy and unit-dose prep.
• Attends codes to provide pharmaceutical expertise and support.
• Monitors medications for patients aged 65+ with reduced creatinine clearance.
• Reviews special care nursery and pediatric meds per policy.
• Checks exchange medication carts before delivery.
• Fills and checks employee prescriptions and counsels staff.
• Reorders stock when low and makes rounds at least twice per shift.

Job Summary And Qualifications

• Assesses all physician orders to insure accuracy, drug allergies, drug-drug interactions, appropriate dose for age/weight of patients.
• Documents all interventions. Communicates well with all health care workers.
• Consuls patients on medication use when requested on discharge of a special care nursery patient, a peds patient and all other patients discharged on seven (7) or greater medications.
• Monitors drugs with a narrow therapeutic range problem prone or high use and makes recommendations to ensure positive patient outcomes.
• Reviews antibiotic therapy for appropriateness of culture and sensitivity reports as well as cost effective alternative recommendations.
• Ensures medications are stored and handled properly by completing unit inspections by the end of each month.
• Assists in teaching programs involving nursing personnel and other health care providers.
• Provides drug information as requested by patients, nurses, physicians and other health care provides
• Directs work of technicians including IV admixture, preparation of intensive care nursery meds, TPN, chemotherapy, and unit dose prepacking.
• Attend codes providing pharmaceutical expertise to the situation.
• Monitors all medications for patients 65 years or older with decrease creatinine clearance.
• Review and prepare all special care nursery and pediatric patients per policy.
• Checks exchange medication carts prior to delivery.
• Fills/checks employee prescriptions ensuring all documentation completed, counsels employee.
• Always reorders when stock is low.
• Makes rounds at least twice a shift - ensures rounds made at least every hour.
• Does a narcotic inventory 100% of the time as per protocol.

What qualifications you will need:

• Graduate of a college of pharmacy with BS or Pharm. D accredited by the American Council on Pharmacy Education.
• Licensed Pharmacist in the Commonwealth of Virginia
• BLS Certification
• Previous experience as a Pharmacist (hospital pharmacy experience preferred)
