Job summary

RADHealth+ is seeking a Pharmacist to join our home infusion team. You will evaluate drug therapy, collaborate with physicians, nurses, and other professionals, and ensure compliance with all laws, regulations, and accreditation standards.

The role requires a strong clinical background and the ability to manage complex home-care caseloads. This position offers a day shift with on-call evenings and weekends as scheduled, plus a weekly on-call rotation and travel requirements of 0%.

Qualifications

  • Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred
  • Registered Pharmacist License in state(s) of practice
  • Minimum of two years of recent experience in an acute-care setting or one year of recent experience in home infusion
  • Experience with compounding infused drugs
  • Proven record of evaluating and monitoring drug therapies for home care patients
  • Good organizational and time-management skills
  • Participation and membership in professional societies and organizations is desirable
  • Excellent verbal and written communication skills
  • Flexible schedule with the ability to work on-call evenings and weekends on an as-scheduled basis

Responsibilities

  • Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
  • Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
  • Adheres to all appropriate standards of accreditation and participate in quality assurance activities.
  • Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution.
  • Reviews, receives and verifies all new patient orders and document on the respective order form.
  • Assists the physician and other involved healthcare professionals with planning and implementing patient care in the home.
  • Setting therapeutic objectives for drug therapy and establishing monitoring parameters for safety and efficacy.
  • Preparting regular progress reports.
  • Conducts and documents initial and ongoing assessment of patient, drug and disease information to identify real and potential drug-related problems.
  • Monitors the outcomes of drug therapy by collecting, evaluating and interpreting laboratory and clinical data.
  • Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
  • Maintains thorough documentation in patient records and logs.
  • Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.
  • Maintains clinical skills by reviewing literature and attending in-services, seminars and conferences.
  • Compounds sterile/non-sterile preparations following aseptic technique where allowed.
  • Ensures clean room areas and equipment are maintained in compliance with company policies and USP 797/800.
  • Documents all activities on appropriate logs.
  • Pharmacists’ duties include overseeing the Pharmacy technician functions as required by Pharmacy Board rules.

Job description

Shift: Day 5*8 (Monday - Friday) (08:30AM - 05:30PM // 08:00AM - 05:00PM // 09:00AM - 06:00PM)


Experience Required: 2+ years


Local Pay: $60-$65/hr


WEEKLY TRAVEL PAY PACKAGE (resides out of 50+ miles)


Weekly Tax Free Stipend to be paid: $1,218.00


Weekly Straight Pay (Taxable): $1,782.00


Weekly Gross: $3,000.00


Job Description:

Scheduled On Call shifts: Friday 5:30 PM - Monday 8:30 AM---typically once a month


Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals.


Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.


Adheres to all appropriate standards of accreditation and participate in quality assurance activities.


Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution.


Reviews, receives and verifies all new patient orders and document on the respective order form.


Assists the physician and other involved healthcare professionals with:


Planning and implementing patient care in the home;


Setting therapeutic objectives for drug therapy;


Establishing parameters for monitoring the safety and efficacy of drug therapy; and


Preparting regular progress reports.


Conducts and documents initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs.


Monitors the outcomes of drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.


Provides consultation and answers inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.


Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians and other healthcare professionals.


Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy.


Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars and conferences.


Compounds sterile mixtures and non-mixtures, including, but not limited to parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics, using aseptic technique where state laws permit.


Ensures compounding/clean room areas and related equipment are cleaned, sanitized and maintained in compliance with company policies and procedures, and USP 797 and USP 800 standards.


Documents all activities on appropriate logs.


Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars and conferences.


Pharmacists’ duties are extended to include overseeing of the Pharmacy technician functions as required by Pharmacy Board rules.

**To perform this role will required constant sitting and typing on a keyboard with fingers, and occasionally standing, and walking. The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs**
