Job summary

Amerita, an affiliate of BrightSpring Health Services, seeks a Pharmacist to join our home infusion pharmacy team. The Pharmacist will evaluate drug therapy, coordinate with physicians and nurses, and ensure compliance with USP standards in a fast-paced home care setting.

Responsibilities include reviewing orders, preparing sterile mixtures, supervising technicians, and documenting clinical activities. On-call evenings and weekends are required, with competitive pay and full benefits.

Qualifications

  • Bachelor of Science in Pharmacy required.
  • Doctor of Pharmacy preferred.
  • Registered Pharmacist License in state(s) of practice.
  • Minimum of two years in an acute-care setting or one year in home infusion.
  • Experience with compounding infused drugs.

Responsibilities

  • Evaluate and monitor drug therapy with physicians, nurses and other healthcare professionals.
  • Adhere to state/federal laws and accreditation standards.
  • Coordinate referrals and admissions with nurses and distribution.
  • Review new patient orders and document accordingly.
  • Assist physicians with home care planning and therapeutic objectives.
  • Assess patient, drug and disease information for drug-related problems.
  • Monitor drug therapy outcomes using lab and clinical data.
  • Provide drug information to patients, families, and clinicians.
  • Maintain thorough documentation of clinical activities and communication.
  • Oversee pharmacy technician functions as required by Board rules.

Schedule:Monday - Friday9:00am - 5:30pm

Weekends and on- call rotations required

Responsibilities
  • Evaluates and monitor drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
  • Adheres to organization policies and procedures and all applicable state and federal laws and regulations governing pharmacy practice.
  • Adheres to all appropriate standards of accreditationand participate in quality assurance activities.
  • Coordinates and facilitate new referrals and admissions through collaboration with nurses, customer service and distribution.
  • Reviews, receives and verifies all new patient orders and document on the respective order form.
  • Assists the physician and other involved healthcare professionals with:
    • Planning and implementing patient care in the home;
    • Setting therapeutic objectives for drug therapy;
    • Establishing parameters for monitoring the safety and efficacy of drug therapy; and
    • Preparting regular progress reports.
  • Conducts and document initial and ongoing assessment of specific patient, drug and disease information to identify real and potential drug-related problems or needs.
  • Monitors the outcomes of drug therapy by collecting, evaluating and interpreting the results of laboratory data, clinical data and other pertinent patient information.
  • Provides consultation and answer inquiries from patients, families, physicians and other healthcare professionals regarding drugs and their use.
  • Maintains thorough and appropriate documentation in the customer/patient record of all clinical activities and communication with patients, physicians and other healthcare professionals.
  • Processes and check all medications, supplies and equipment prior to dispensing from the pharmacy.
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars and conferences
  • Compounds sterile mixtures and non-mixtures, including, but not limited to parenteral nutrition, antibiotics, chemotherapy, hydration and narcotic analgesics, using aseptic technique where state laws permit
  • Ensures compounding/clean room areas and related equipment are cleaned, sanitized and maintained in compliance with company policies and procedures, and USP 797 and USP 800 standards
  • Documents all activities on appropriate logs.
  • Maintains clinical skills and knowledge of pharmacotherapy by reviewing literature and attending in-services, seminars and conferences
  • Pharmacists’ duties are extended to include overseeing of the Pharmacy technician functions as required by Pharmacy Board rules
Qualifications
  • Bachelor of Science in Pharmacyrequired; Doctor of Pharmacy preferred
  • Registered Pharmacist License in state(s) of practice
  • Minimum of two (2) years of recent experience in an acute-care setting or one (1) year of recent experience in home infusion therapy
  • Experience with compounding infused drugs
  • Proven record of evaluating and monitoring drug therapies for home care patients
  • Good organizational and time-management skills
  • Participation and membership in professional societies and organizations is desirable
  • Excellent verbal and written communication skills
  • Flexible schedule with the ability to work on-call evenings and weekends on an as-scheduled basis
  • Percentage of Travel: 0-25%-Minimal travel may be required

**To perform this role will required constant sitting and typing on a keyboard with fingers, and occasionally standing, and walking. The physical requirements will be the ability to push/pull and lift/carry 10-30 lbs**
